Assists caregivers of any age that care for individuals 60 years of age and over. Caregiver Specialist Caseworkers can provide information and resources to both the caregiver and the care receiver.

Offers a disaster program that consists of several basic services. These services address many of the typical needs of a disaster survivor. Salvation Army disaster relief is also flexible according to the magnitude of the disaster.

Provides a team of trained volunteers who can mobilize resources to provide post-disaster mental health services. This team is sponsored by the Department of Human Services, Northeast Iowa Behavioral Health and County Social Services.

Provides housing stability support for residents of Stephenson and Jo Daviess Counties who are at risk of eviction or losing essential services. Through federal and state funding, can help local households with rent, mortgage, or utility payments to prevent homelessness.

Assistance is not guaranteed and is based on available funding. Priority may be given to households with children, seniors, or individuals with disabilities.

Patients in supportive care may receive visits or phone calls from a nurse practitioner, a nurse, or a social worker. Supportive care offers visits at FHN offices and at FHN Memorial Hospital, in long-term care facilities, and in the home for those unable to travel to the clinic due to physical disabilities or distressing symptoms.

Provides library services. Items that may be borrowed from or used in the library include books, music recordings, videos and DVDs, audiobooks; computer software, magazines, and newspapers. The library also has computers and WiFi for public access. There are also library sponsored programs for all ages offered.

Air conditioners provided, when available, to the most vulnerable individuals (especially seniors 60 and older) who are experiencing health problems that make air conditioning a medical necessity, or those of any age with medical necessity.

Provides probation services to residents of Kankakee County, Illinois. Services include supervision of adults and juveniles, information and support with the legal system, help with other social agencies, access to individual and group counseling, and an abuser treatment program. 

Provides abuse and crisis intervention services to those going through a domestic violence, sexual assault, stalking, or sex trafficking situation. Services include a family violence crisis line, counseling or counseling referrals for survivors of domestic violence or sexual assault, emergency shelter and transportation, peer support groups, and child advocacy.

Protection order assistance, criminal justice/legal advocacy, and medical advocacy are also available to those who may need it.

Works to build awareness of abuse by partnering with local agencies to offer community education programs and a resource library.

Origin to destination service. If rider needs assistance boarding or alighting, client must let RMTD know at the time the ride is scheduled.

Riding Rockford ParaTransit is equivalent to riding RMTD's fixed route system. There is a scheduled arrival time and rider must be ready when the vehicle arrives. The driver can wait only 5 minutes for the rider to board.

The vehicle may arrive within 15 minutes of the scheduled pick-up time. If the vehicle has not arrived 20 minutes after the scheduled pick-up time, call for assistance.
