Offers home-based options with a weekly visit from a teacher in the family's home along with two special days a month for activities with other families.
Plains Area Mental Health - Denison Outreach and Den
Supported Community Living and Home Based Community Services to provide the support necessary to assist adults with severe and persistent mental illnesses to live, work and enjoy recreational opportunities in community settings and live as independently as possible.
Community Support Services include monitoring mental health symptoms, transportation, supportive relationships, communication with other providers, assistance in attending appointments and obtaining medication, and crisis intervention.
Day Habilitation programs provide daily structure and positive support to help clients establish relationships.

FHN Family Healthcare Center - Lanark
Patients in supportive care may receive visits or phone calls from a nurse practitioner, a nurse, or a social worker. Supportive care offers visits at FHN offices and at FHN Memorial Hospital, in long-term care facilities, and in the home for those unable to travel to the clinic due to physical disabilities or distressing symptoms.

Haven House - South Sioux City
Provides abuse and crisis intervention services to those going through a domestic violence, sexual assault, stalking, or sex trafficking situation. Services include a family violence crisis line, counseling or counseling referrals for survivors of domestic violence or sexual assault, emergency shelter and transportation, peer support groups, and child advocacy.
Protection order assistance, criminal justice/legal advocacy, and medical advocacy are also available to those who may need it.
Works to build awareness of abuse by partnering with local agencies to offer community education programs and a resource library.
Rockford Mass Transit District
Origin to destination service. If rider needs assistance boarding or alighting, client must let RMTD know at the time the ride is scheduled.
Riding Rockford ParaTransit is equivalent to riding RMTD's fixed route system. There is a scheduled arrival time and rider must be ready when the vehicle arrives. The driver can wait only 5 minutes for the rider to board.
The vehicle may arrive within 15 minutes of the scheduled pick-up time. If the vehicle has not arrived 20 minutes after the scheduled pick-up time, call for assistance.
