Carle - Community Health Initiatives
Provides Community Health Workers (CHWs) who partner with community agencies to help individuals living with chronic health conditions within underserved communities. CHWs assist clients in overcoming barriers to health, social services, education, and employment through client visits and community engagement.
Services include:

North Fayette Valley Community Coalition
Provides information to coalition and community members about the substance use problem in the community, state, and country. Information about the consequences of alcohol and/or drug use and addiction the incidence and prevalence of specific drug use. Information is made available through a variety of formats including trainings/workshops, publications, FaceBook postings, and staff presentations to community groups and schools.
In addition, the Coalition provides parents and caregivers with information and resources they need to address alcohol and other drug use with their children including: Tips for talking with children about substance use, what to look for with suspected use, consequences of youth substance use, understanding why children start using drugs, and resources to answer tough questions. The coalition offers parent education classes and can provide help in developing Family Substance-Free Pledges.

Provides fire protection to the City of Freeport. Service includes business safety education, industry safety education, fire safety for senior citizens, juvenile fire setters' program, fire prevention week education in the schools, fire station tours, home inspections, disaster planning in the home, a blood pressure check program, a smoke detector program, and fire investigation.
Henderson County Health Department
Provides services to individuals who have conditions such as congestive heart disease, kidney disease, diabetes, muscle-nerve problems or respiratory diseases and are able to be discharged from a hospital or nursing home but need additional care at home. Require short term assistance at home because of same day or outpatient surgery or maternity-related incapacity.

City of Omaha Planning Department, Housing and Community Development Division
Eliminates and stabilizes lead-based paint hazards and other lead related hazards for single-family and multi-family residences. Includes both interior and exterior lead paint hazards.
The following repairs are eligible:
- Replace windows and door with lead-based paint
- Stabilize or remove lead-based paint on porches
- Stabilize deteriorated lead-based paint on the interior and exterior of the home
- Replace soil that contains high levels of lead
HOMEOWNERS must meet the following guidelines:
- House is located in the targeted area
- Household has children age six or under; priority is given to children with elevated blood levels of 5 ug/dl or higher
- Income is at or below 80% of HUD median family income
INVESTOR-OWNERS must meet the following guidelines:
- Household has children age six or under; priority is given to children with elevated blood levels of 5 ug/dl or higher
- Rent to income eligible households and agree to affirmatively market units to households with young children for a three-year period
- The unit must be currently occupied
Funding is in the form of a grant or secure deferred payment loan (DPL) meaning that a lien is placed on the property for 36 months. The lien is released after three years if all loan requirements are met, and no repayment is necessary.

Hardin County Family Services and Emergency Food Pantry
Offers a food pantry to those in need. Services may also include hygiene and cleaning products in most of the food pantries, including diapers, soap, shampoo and conditioner, feminine hygiene products, and other essentials. Food delivery is available upon request.
Mahaska County Offices - 1st Street
Collects property taxes, conducts annual tax sale for delinquent taxes, processes vehicle transactions, issues driver's licenses, and issues motor vehicle titles and registrations.
Winneshiek Medical Center
Provides care for sexual assault victims. Nurses provide emotional support, a physical examination and wellness check. Will also collect medical-forensic evidence, assist with reporting the crime to police, when requested (mandatory reporting for children), assist with concerns about sexually transmitted infection and pregnancy, and development of a medical follow-up plan.
Kreider Services
Provides day services for individuals with developmental disabilities. Services include social, vocational, personal, and academic skills trainings for adults with intellectual developmental disabilities. Services occur both onsite and, in the community, and community employment is also an option for eligible individuals.
