The mental health team works with patients to assess current coping skills, address emotional needs, and work through lifelong patterns. A board-certified psychiatric mental health nurse practitioner provides specialty care in the areas of psychiatric consultation, evaluation, and psychiatric medication management. These services are provided on-site by the mental health team.

The goal is to make it easier for individuals to get help to stay healthy, feel better, and recover. It also makes sure individual's with mental health or substance use problems can get the same kind of help as those with other health problems. All doctors and providers work as a teak so care is organized.

Integrated Health Home services are those services provided to adults with serious mental illness (SMI), and Medicaid coverage.
Services are classified as whole person care, for physical health, mental health, and lifestyle choices that impact overall wellness. Services are provided by an Integrated Health Home Team which includes a nurse care manager, a care coordinator, and peer support specialists.
Integrated Health Home providers help with the following:
- Manage physical and behavioral health services;
- Create a whole health plan for medical care and wellness goals;
- Focus on choices, strengths, good health, and total wellness;
- Help with transportation to healthcare appointments if needed;
- Use technology to link all your services together; and
- Provide peer support services to encourage recovery and resiliency.

Offers behavioral health assessment and diagnosis, family therapy, individual psychotherapy, child therapy, marital therapy. Also offers psychiatric nurse practitioner services including behavior health assessment, medication management and brief psychotherapy.
Children's Square Campus
Provides in-home, skills-based interventions designed to help children and families develop healthy coping strategies and improve daily functioning. Empowers children and their caregivers with practical tools to manage behavior at home, in school, and in the community.
Focuses on individualized interventions for both the child and their family:
- Child Interventions: Address psychological, emotional, behavioral, cognitive and social factors impacting the child’s well-being.
- Family Interventions: Strengthen family dynamics and communication to support the child’s growth and success.
Services are provided in the child’s home (with family present) through weekly individual and/or family sessions, based on the family’s availability.

Community Support Services (CSS) provides workers in the individual’s home and community to help overcome barriers to mental health and healing. Community Support Advocates
Provides outpatient mental health therapy, medication management, and psychiatric assessments. Services are offered to adults with disabilities, including individuals with Mental Illness, Intellectual Disability, or other Developmental Disabilities, who require support in multiple areas.
The program integrates Medicaid funding and county regional funding to assist individuals in transitioning back into the community after hospitalization for acute or chronic mental or emotional disturbances.
Eligibility requires qualification for Polk County Regional funding; individuals may or may not be eligible for Medicaid.

Integrated Health and Community Outreach Programs are offered, including Jail Diversion programs, Integrated Health programs, and Support Community Living programs. Life skills education is provided to help individuals develop the knowledge and skills necessary for independent living, addressing areas such as job search, money management, nutrition, home management, and health care.
