Provides transitional living for young women and mothers with up to two children who have aged out of foster care or otherwise experiencing homelessness.

Participants enrolled at CHH enjoy private bedrooms, full sized kitchens, family style living spaces, and large outdoor recreational areas. While residing at CHH, participants work alongside Program Specialists and an onsite LIMHP/LADC to help them achieve their individual goals. Evening classes and therapy groups follow the 8 Dimensions of Wellness model as participants work toward increasing their independent living skills, enhancing their mother/child relationship(s), and caring for their mental health.

Offers a variety of services to address the full range of housing needs Veterans may have. Services include:

Prevention services such as Supportive Services for Veteran Families that keep Veterans from becoming homeless.

Residential services such as the Health Care for Homeless Veterans Contracted Residential Services or Grant and Per Diem programs provide temporary placement, in the form of emergency or transitional housing, for Veterans who need a place to stay right now.

Permanent housing services such as the Supportive Services for Veteran Families and HUD-VA Supportive Housing, or HUD-VASH, programs connect Veterans to affordable housing in their communities. These services come with either short or long-term rental subsidies, case management, and varying levels of wrap-around supportive services to ensure that Veterans have all the necessary resources to stay housed.

VA also has programs to assist Veterans in the criminal justice system with accessing VA services, help Veterans gain meaningful employment, or take care of their health needs through primary care tailored to the unique access barriers that Veterans experiencing homelessness face.

Long-term shelter program for two-parent families, single-parent families, couples of the same and opposite sex, and single cisgender or transgender women experiencing homelessness or near homelessness. Participants in the program are assigned to a Case Manager who works with the client on budgeting, connecting with services, and helping with goal setting.

Transitional housing for homeless single veteran men.

Supportive services include therapy and case management.

Up to 18 months of residential housing for expectant or parenting youth ages 16-21 experiencing homelessness, have run away, or are aging out of foster care.

Living accommodations, independent living skills training, educational opportunities, counseling, advocacy, referral services, and mentoring of youth in a person centered approach.

Each individual's innate talents are identified and shaped to empower the individual with the tools necessary to become a productive member of society.

Parenting classes and other child bases serves are also provided.

Oversees care for adults with serious mental illness. Services include case management, psychiatry, and medication management, as well as outreach for everything from job training to primary healthcare to daily life skills.

Assists homeless teens with a safe, stable home, provides income adequate to meet their basic needs, and assists and directs in overcoming the problems that interfere with their ability to live independently.

- Group therapy.

- Psychiatric services.

- Intensive community-based treatment.

- Crisis services.

Oversees care for adults with serious mental illness. Services include case management, psychiatry, and medication management, as well as outreach for everything from job training to primary healthcare to daily life skills.

Provides abuse and crisis intervention services to those going through a domestic violence, sexual assault, stalking, or sex trafficking situation. Services include emotional support and crisis counseling, lethality assessment and safety planning, case management, children's advocacy, short term mental health counseling, health care navigation, and a support group.

- Rapid Rehousing.

Offers residential treatment programs for adults with substance use and co-occurring mental health disorders. Programs serve men and women, women with children, young adults, veterans, and essential and frontline workers. Also offers medically monitored detox and withdrawal management.

Sober living recovery homes and halfway houses provide wraparound support for individuals in early recovery. Also offers supportive housing for individuals living with mental health disorders.

Adult outpatient services include therapy, specialty care such as TMS therapy and MAT, family support and education, psychiatric care, intensive community-based case management, and fulfillment of legal behavioral healthcare requirements related to court systems.

Offers transitional housing and support services for Veterans experiencing homelessness. Specific case managers to assist veterans in finding affordable housing, access to appropriate health care, substance abuse services, and mental health recovery services.

Provides a safe place for at-risk and homeless youth (ages 16-24) in the Polk County area to connect with services. Services include street outreach, drop-in services, daily meals, emergency beds, an art studio, a computer lab, and mental health counseling options. 

The center also offers connections to additional YSS supportive housing programs, such as Lighthouses, Transitional Living Program, and Rapid Re-housing. Supportive services may include educational opportunities, life skills training, financial planning, and counseling to help youth and young adults, including single mothers and their infants, achieve self-sufficiency and progress toward their goals.

Provides emergency shelter and longer-term housing opportunities and supportive services for single adults and families experiencing homelessness and extreme housing instability.

Offers a transitional living center for homeless veterans who want a chance to rebuild their lives. Many veterans enroll after undergoing intensive substance abuse treatment. Clients must undergo a screening process and abide by all rules while a part of the center. Graduates are encouraged to enroll in TLS-Aftercare, a program that offers ongoing support and crisis intervention within the community.

Supportive Housing Program is a HUD-funded transitional living program for youth ages 18-24. Youth are provided with a subsidized house/apartment and work with a case manager to reduce homelessness or unsafe living conditions and to achieve permanent housing. Supportive Housing serves both single youth as well as pregnant and/or parenting youth. Youth can remain in Supportive Housing for up to 24 months.

Permanent Housing Program is a HUD-funded program for youth ages 18-24. Youth are provided with a subsidized house/apartment and work with a case manager to reduce homelessness or unsafe living conditions and to achieve permanent housing. Permanent Housing serves youth who have a mental health and/or substance abuse diagnosis. Permanent Housing serves both single youth as well as pregnant and/or parenting youth. There is no time-limit to their participation.

Life skills training in the areas of education, employment, independent living, and parenting.
