Offers a holistic approach that ensures participants receive comprehensive and integrated care including the mind-body connection, while addressing their support needs.
Each participant is assigned a team of professionals including a Registered Nurse, Care Coordinator, and Peer Support (adult) and Family Peer Support Specialists (children) who actively review the member’s mental and medical condition and create an individualized care plan. Provides assistance in navigating the healthcare system and accessing resources, personalized supports, and a major focus on wellness, prevention activities, education, and healthy lifestyle choices.
