Open to families who are in danger of eviction, foreclosure, or homelessness or are currently homeless

Photo ID, Social Security cards for all members of the household, proof of address, proof of 30-day gross income, current landlord statement with the following information: tenants name and address, amount of rent per month, months individual is behind/owes, total amount owed without late fees, landlord's name, address, phone number, and signature, in addition, customer must provide a written statement explaining their need for assistance, additional information may be requested at the time of appointment
